Person + had + Past participle.

She had thought a lot about it.

Negative

Person + hadn’t + Past participle.

She hadn’t made a decision.

Interrogative

Had + Person + Past participle?

Had she talked to someone before?

Examples:

When my sister arrived home:

I had done my homework – I hadn’t done any exercise

I had finish dinner – I hadn’t cooked for tomorrow

I had taken a bath – I hadn’t straightened my hair

I wondered:

Had she driven home?

Had she brought a present for me?
